Max Payne, un honnête policier de New York, semble vivre le rêve américain : marié à une femme qu'il adore, Michelle, avec qui il vit dans une charmante maison de banlieue, il est également père de famille depuis peu. Mais le rêve bascule en cauchemar lorsqu'un soir, alors qu'il rentre chez lui, Max découvre des voyous dans sa demeure sous l'emprise d'une nouvelle drogue, la Valkyrie (surnommée la V). Les voyous sont abattus mais le drame est là : sa femme et son bébé viennent d'être violemment assassinés « le temps d'une minute new-yorkaise ».

S'enfonçant dans la dépression, Max Payne accepte de prendre une couverture dans le milieu mafieux que lui propose son ami Alex Bolder afin de démanteler le réseau de distribution de Valkyrie. Trois ans plus tard, alors qu'une tempête de neige d'une violence extrême immobilise New York, Max est toujours infiltré dans les pires familles mafieuses et n'a pas vraiment avancé mais son contact B.B. l'informe que son ami Alex a de nouvelles informations et l'attend dans le métro, station Rosco Street. Sur les lieux, Max se retrouve dans une fusillade sanglante qui ne se terminera que trois jours plus tard l'amenant à livrer seul une guerre contre le réseau mafieux et à trouver la vérité sur l'assassinat de sa famille.

The game’s protagonist is Max Payne. His family was murdered by a group of drug addicts high on a new drug called Valkyr. He then switches from Homicide Detective to a DEA agent working to put the Punchinello family, who are responsible for the drug’s distribution, in prison. After he is framed for the murder of a friend in the DEA, he begins working to clear his name.

The game’s antagonist is Nicole Horne. She is the one who is responsible for Valkyr’s existence, as she led the government team that invented it. It is revealed late in the game that Angelo Punchinello, the head of the Punchinello family, is being controlled by Horne.

Another prominent character is Alfred Woden. The head of an Illuminati organization called the Inner Circle. He and the Inner Circle offer to clear Max’s name in exchange for the death of Horne. Mona Sax is the sister of Angelo Punchinello’s wife. Mona seeks to kill Angelo, and after his death, she joins with Horne. She finds herself unable to kill Max after Horne instructs her to do so.
